The state of Indiana allows for a medical or religious exemption (not a philisophical one) for vaccinations. My daughter will be going back to her private Christian school for Kindergarten soon and we are choosing to opt out of her 4-6 year old shots.

Just wondering if anyone has ever used a religious exemption and, if so, did you make up your own form or did you use one that you obtained from elsewhere? Also, did you have any problems with the school accepting the exemption and/or refusing to allow admission to school? Thank you for any help you may have to offer.
