They don’t, or at least most of them don’t. I doubt most TERFs are true believers.
All they know is that anorexics view their body in a vastly different light than it actually is, and we don’t give them weight loss surgery or anything. They think/pretend that trans people are the same in the first part, so they use this analogy to push against gender-confirming medical work.

When you say ‘similarities,’ are these similar alterations, or just alterations in the same general area? I mean, at what distance do you need to stand so they look similar?

Agreed about the ignorant comparisons of us with anorexics, but as someone who’s seen way too much of what TERFs say… I assure you that the vast majority of them are the truest of true believers. Yes, even if they say something that reads as insane/ridiculous, you better believe it: they mean every word.
